Shipping methods of Will Call and Freight Quote

I'm posting this here and then also as a question to Zoey. I'm sure I will not be the only one to need this going forward. When answered I'll post the solution here.
Right now I have "Free Shipping" allowed for orders "Over" $149 of products with the attribute of "Free Shipping" equaling "YES".
We do this because some items we sell are very large and heavy, we cannot offer free shipping on these items.
What we need are two other shipping methods.
1) Will Call - This is for customers who may live in the area and want to drive over and pick up items rather than pay shipping. We need this to come up "ONLY" for people who have a shipping address in California. Right now when I make this via a table based shipping method or discount rules the "Free Shipping" method comes up with it so people choose the free shipping. The instructions on the support site are not clear on the table based method.
2) Freight Quote - We would like this to be an option for people who want to order a quantity larger than 5 products which are listed with the attribute of "Free Shipping" = "NO". This would be available to orders shipped to any location. These orders could also include items with the attribute of "Free Shipping" = "YES" but must have the qualifying 5 items of "NO".
So any of you out there already doing this or have done some of this???

What I have found is that you need to use Table Rates and rename as the "Will Call" and "Freight Quote".
The only thing is that they will show up for every orders shipping options. I just went ahead and made it clear it was only for certain items in larger quantities.
