Adding Custom Options at Checkout

Store owners should be able to create custom drop down boxes at checkout for Customers to select. This could have a number or uses. I would like to give customers options for what charity their purchase will support, but this could also be used to gauge customer satisfaction (rate your experience: 1-5) or a number of other uses.
