Add Custom Fields to Checkout

Was checking out of a competitor's store and liked how their checkout had the option for "Order Options" where I had to answer yes or no (questions) to proceed.
Example: If an item is out of stock or the full quantity is not available would you like us to contact you? (Y/N)
In addition to custom fields, the backend ability to assign this to: ALL products, specific categories or specific skus.
